Group Schedule Information Form
Use this form to submit the required information to add a new group to the MWVANA schedule
or to update schedule information for an existing group. Verification of online submissions is necessary and a contact method,
email or phone, is required to process a request.
1. Additions/Changes must be received in writing via this electronic form or the printed form available at MWVANA Service Committee meetings (also available for dowload here:
schedule_form.pdf)
2. The deadline for information to appear on the printed MWVANA schedules is ten (10) days prior to the next monthly MWVANA Area Service Committee meeting. Submissions after the print deadline will be available on the MWVANA web site but will not appear on the printed schedules until the following month.
3. The group is responsible for the accuracy of the information that is submitted.
4. Please make every effort to submit written forms at the MWVANASC meeting. If this is not possible, please contact MWVANA Public Relations to make other arrangements.
